The Referral Process

Initial Contact

We encourage families, healthcare professionals, and potential residents to reach out to us directly. Our knowledgeable team is available to discuss our services, answer questions, and guide you through the initial steps.

Comprehensive Needs Assessment

Upon receiving an inquiry, we conduct a thorough assessment to understand the individual’s specific requirements. This involves gathering detailed information about their health, personal preferences, and any other pertinent details to ensure we can provide the best possible care.

Personalized Care Planning

Based on the assessment, our team collaborates with the individual and their support network to develop a customized care plan. This plan outlines the services and support mechanisms that will be put in place to meet the resident’s needs and promote their independence.

Ongoing Evaluation and Support

Our Assessment Process

At Banstead Care Home, our dedicated and experienced Assessment Team oversees a comprehensive, needs-based assessment process. The Lead Assessor and Service Manager work together to conduct a thorough evaluation, focusing on compatibility, individual needs, and any potential risks. Whenever appropriate, the process involves input from the individual being referred, their support teams, professionals, and families to ensure a holistic understanding of their situation.

We strive to make the assessment process as meaningful and comfortable as possible for everyone involved.

Once the assessment is complete, our Assessments and Operations Team will determine whether a placement can be offered. If a placement is available, our Business Development Team will coordinate the offer and provide support through the next steps.

In cases where we cannot offer a placement, we will ensure that the referrer receives a clear explanation of the reasons why we are unable to meet the individual’s needs at that time.

Next Step

Proposal and Funding

After completing the assessment, if we are confident that we can meet the individual’s needs, we will prepare and submit a detailed proposal to their Health and Social Care Team. This proposal outlines the levels of support we will provide, the methods of delivery, any required multi-disciplinary input from our internal team, a written assessment, and a comprehensive cost breakdown.

Once the proposal is submitted, the Commissioning Team and the Social Worker or Care Manager will review and decide whether the placement can be approved and funded. We maintain regular communication with everyone involved during this period to keep them informed.

Once funding is agreed upon, we plan the transition into the new home and support environment. Our priority is to make this process as smooth and stress-free as possible, ensuring a positive and successful placement for the individual.
